(1)2-inch color bareheaded electronic photo, white background, noless than 295*413 pixel size, 100-200KB in JPG format. Please makesure to upload the correct photo, which will be used later for governmentsystem check;

(2)Scanned copy of valid passport's information page(valid for 18months), current visa/residence permit page (valid for 90 days,studentscurrently in China must provide);

(3)Scanned copy of Senior High School Graduation Certificate orPre-graduation Certificate issued by senior high school (Only availablefor fresh graduates).An official seal is required.

(4)Scanned copyof transcript of all courses during the highestacademic degree education (Transcripts in languages other thanChinese or English shall be attached with notarized translations inChinese or English);

(5)Scanned copy of Non-criminal Record (Valid for 6 months);(6)Scanned copy of Foreigner's Physical Examination Form completed

in Chinese or English;(Valid for 6 months);

(7)E copy of handwriting signature;

(8)Certificate ofLanguage proficiency;

(9)English self-introduction video (3-5 minutes, no more than 50M);(10)Other supportive materials if have, such as excellent performance certificates, etc.

School ot Internatlonal Educatlon of Xuzhou Medical University will select the outstanding applicants according to application materials andadmission scheme. After complete the online application, please pay attention to the message of the apply system and your personal e-mail message to confirm the notification in time.The E-offer (pre-admissionletter) will be sent to the applicant who passed pre-check through thesystem. After that, applicants should pay CNY 5,200(non-refundable forpersonal reasons) for locking the seat and upload the scanning copy ofpayment receipt to the application system within 2 weeks after he/she received pre-admission letter. The admission deposit will be used as the registration fee and the insurance fee during the student's study in China after registration. Failure to pay on time will be regarded as automatic waiver of enrollment qualification.
