5. "Foreigner Physical Examination Form" (valid for no more than half a year at the time of application)

6. Copy of passport

7. No criminal record certificate

8. "Guarantor's Guarantee Letter"

9. Identity proof of the guarantor

10. Electronic version of ID photo (in jpg format, size between 100kb - 600kb)
